چکیده

A 79-year-old man was being evaluated for possible mitral valve repair for increasing mitral regurgitation. Transesophageal echocardiography with color flow Doppler showed left atrial enlargement, mitral valve prolapse, and spontaneous changes in the direction and configuration of regurgitant jets (during the same imaging study) determined by which leaflet prolapsed into the left atrium during systole. A, Mitral valve prolapse (white arrows) involving both leaflets. B, Centrally located mosiac jet (white arrowheads) resulting from the bileaflet prolapse shown in A. C, Eccentric, laterally located mosaic jet along the lateral wall of the left atrium (white arrowheads) associated with prolapse of only the anterior mitral leaflet. D, Eccentric, medially located mosaic jet (white arrowheads) along the posterior wall of the aorta associated with prolapse of the posterior mitral leaflet. LA indicates left atrium; LV, left ventricle; and AO, aorta.

in the Evaluation of Mitral Valve Prolapse

we studied 49 consecutive patients in whom the sites of mitral valve prolapse were confirmed at the time of operation. The study group consisted of 22 patients with anterior leaflet prolapse, 24 patients with posterior leaflet prolapse, and three patients with multiple scallop prolapse (one patient with both anterior leaflet and middle scallop prolapse, and two patients with both medial and lat...

Mapping of mitral regurgitant defects by cardiovascular magnetic resonance in moderate or severe mitral regurgitation secondary to mitral valve prolapse

PURPOSE In mitral valve prolapse, determining whether the valve is suitable for surgical repair depends on the location and mechanism of regurgitation. We assessed whether cardiovascular magnetic resonance (CMR) could accurately identify prolapsing or flail mitral valve leaflets and regurgitant jet direction in patients with known moderate or severe mitral regurgitation. METHODS CMR of the mi...

The edge-to-edge technique for mitral valve repair

The edge-to-edge technique was introduced in the surgical armamentarium of mitral valve repair in 1991 and has progressively been used to restore mitral competence in the setting of degenerative, post-endocarditis and functional mitral regurgitation. Appropriate indications and awareness of the important technical aspects of the procedure are prerequisites for a good outcome. The free edges of ...

The edge-to-edge repair (EtER) technique was introduced in 1991 to treat both mitral regurgitation (1) and tricuspid regurgitation (2) caused by complex valvular lesions. The method involves anchoring the free edge of the diseased leaflet to the corresponding edge of the opposing leaflet, just at the place of the regurgitant jet. When the middle sections of the leaflets are sutured, a ‘double-o...

Towards comprehensive assessment of mitral regurgitation using cardiovascular magnetic resonance

Cardiovascular magnetic resonance (CMR) is increasingly used to assess patients with mitral regurgitation. Its advantages include quantitative determination of ventricular volumes and function and the mitral regurgitant fraction, and in ischemic mitral regurgitation, regional myocardial function and viability. In addition to these, identification of leaflet prolapse or restriction is necessary ...
